Lean and friendly Dr. Robin Tattershall, above, offers a different kind of vacation at his Bougainvillea Clinic, left, overlooking Road Town, Tortola, in the British Virgin Islands. You can, if you meet the doctor's approval, come away tanned, fit and with a new face or a different nose or figure. The clinic with its pool and Moorish arches was built to be a small, posh hotel.
